The symptom quiz

The quiz loads a script and embedded page from QuizFlow Labs. The published question set does not request a name, email, or other lead contact information. Your symptom responses are processed by QFL, rather than a SIBOQuiz database. Not requesting contact information does not guarantee anonymity.

QFL may process technical information such as IP address, browser details, and interactions. Its data-processing terms describe quiz responses as information it may process. We have not independently established the precise response-retention period for this public quiz. Please avoid using it if you are not comfortable with that processing.

Site operation

This revised site does not offer patient accounts, practitioner listings, or a lead-contact form. Hosting infrastructure may maintain ordinary security and request logs. Removal of these features does not itself delete records or backups held by the previous service.

Optional analytics

Google Analytics is off unless you choose to allow it. A banner offers equally prominent options to allow analytics or decline when you have not made a choice. Your choice is remembered in this browser, and the footer controls let you change it later. With your permission, Google Analytics measures page visits across this site, including the homepage, learning pages, quiz page, and Gut Health Guide page, plus practitioner template referral clicks. Visiting a page is measured separately from using its embedded quiz or guide. This site's analytics code does not send quiz answers, scores, risk labels, guide conversations, email addresses, free-text searches, or lead identifiers to Google Analytics. Page-view URLs contain only known site paths; query strings, URL fragments, and referring URLs are excluded. Page titles come from fixed site metadata.

We store your analytics choice in this browser. Analytics stays off until you allow it. Visitors who previously allowed practitioner-only analytics are asked to choose again for site-wide measurement; previous refusals remain respected. If you allow analytics on the published canonical site, Google may set analytics cookies and process technical information such as browser and device details and IP addresses used to derive approximate location. Google Signals and advertising personalization are disabled in our tag configuration. You can turn analytics off in the footer; this clears this site's Google Analytics cookies and stops future analytics on subsequent page loads, but does not delete data already collected by Google. QFL's own processing is governed separately; these controls do not manage third-party storage inside its quiz.

Practitioner referrals

Commercial template links include fixed campaign parameters identifying SIBOQuiz.com and the link placement. They do not include symptom or patient information. QFL may associate these campaigns with a practitioner account created after following a referral.

Amazon affiliate links

Optional resource links use Amazon affiliate tracking to credit qualifying purchases to the operator. We do not include quiz answers or scores in these links. No Amazon product widget, image, or tracking script loads on this site; following a link opens Amazon, where its own privacy and cookie policies apply.

Requests and corrections

For privacy questions, access or deletion requests, or a concern about a previous record, contact the operator through Innovative Edge Consulting. Do not include symptom answers in an initial message.
